Skilled Home Health care can help people achieve optimal health and independence at home during a recovery from surgery, injury or illness, managing a serious or chronic disease, or dealing with multiple diagnoses.

What is Home Health Care?

Home Health services include in-home nursing and therapy services for those recovering from illness or surgery, or managing a chronic condition – at home. You’ll receive the services you need based on an individual plan of care developed by our clinical team and your doctor: Some of the home health care services we offer include:

Who is eligible for Home Health Care?

To be eligible for home health care, you must have a medical condition that meets the criteria for home health care. Doctor’s orders are needed to start care. Once your doctor refers you for home health services, we will schedule an appointment to come to your home to talk to you about your needs. We will work alongside your doctor to establish a care plan and keep your doctor updated on your progress. Ask your doctor about home health if you or a loved one want to continue living independently while receiving care for injury or illness.

Who provides Home Health Care?

Our services are provided by licensed professionals. All of whom are trained and background-checked for your security and peace of mind.

Who pays for Home Health Care?

Home health care is usually covered by Medicare and Medi-Cal. Services are also often covered by private insurance carriers. Once we receive a referral from your doctor, we’ll contact your insurance company directly to determine your coverage. Where do you care for patients?

“Home” means where you live. This could be a personal residence, assisted or independent living community, nursing home, or wherever you call home.

What does “homebound” mean?

Medicare defines “homebound” status as being those patients that require assistance when leaving the home and that when they do, it requires a considerable taxing effort.

How long will I receive home health care?

Your doctor will determine the number of visits you receive, how often the visits should occur and how long they should last, based on your needs and health status. Some patients require several weeks of care while others may remain on service for several months. Patients are discharged from home health care services when the individual goals on the plan of care are met.

Is Mission Healthcare a Medicare-certified home health agency?

All of our locations are Medicare-certified and we are a long-standing member of the National Association of Home Care.

When will my home health visits start?

A nurse or therapist will contact you to schedule the first visit. If you’re coming from a hospital or nursing facility, the initial visit will usually happen within 24 hours after you’re discharged to make the transition easier.

My doctor is ordering home health care for me; Can I request Mission Healthcare?

Absolutely, we’d be honored to care for you or your loved one. Federal laws give patients the freedom to choose their health care provider.

Who would benefit from home health care?

You or a loved one may benefit from home health care if you:

  • Have a chronic illness such as heart disease, diabetes, kidney disease or COPD
  • Are recovering from surgery, hospitalization or illness
  • Need help to get out of the house
  • Frequently visit your doctor or hospital

Is hospitalization a requirement to receive home health care?

No. Hospitalization is not a requirement to receive home health care. Many patients are referred by their doctor.

What is the role of family members in home health care?

When you choose home health care you can remain in your home, surrounded by friends and family and still receive the expert care that you need to help you recover. Family members can visit as much as necessary. They can have peace of mind, knowing that you are receiving care in a safe, therapeutic environment.

Who comes into my home to provide care?

Depending on your specific needs, the caregivers and clinicians coming into your home can vary. You could have different caregivers at different times of the day, week, or month as part of your care. Personal care and companionship services are most often provided by a home health aide (HHA) or certified nursing assistant (CNA). Private duty nursing care and home health care services are provided by registered nurses (RNs) or licensed practical nurses (LPNs), physical therapists (PTs), occupational therapists (OTs), or speech language pathologists (SLPs). A medical social worker (MSW) can help individuals and family members connect with community resources.

Additionally, depending on the provider you choose, there may be other professionals who don’t come to your home on a regular basis, but can be instrumental in helping to manage your care. These experts may help with things like overseeing your overall care experience, coordinating schedules, billing insurance companies and Medicare directly, and verifying your benefits eligibility.
